A transport company has faced fines after breaching the Work Health and Safety Act 2011 - where they failed to report an employee's notifiable incident to Workplace Health and Safety Queensland (WHSQ) immediately.
In fact - the WHSQ weren't known to the incident until more than 12 months after the accident happened - when the injured worker came forward.
